GTX960: NVIDIA driver crashing/recovering, and monitor flashes black

I build my PC a little over two months ago, and have been having issues in the last few weeks. It started with my NVIDIA driver crashing and recovering, where my screen will go black for a couple seconds before coming back. This typically happens when playing WoW. This went from happening once every other day or so, to once a day, to a couple times a day. Now, sometimes when I'm playing games or watching videos, my monitor will go black for a few seconds (without referencing any driver issues like it used to) and come back, with my monitor showing the HDMI input in the corner. Specifically, I've been watching Game of Thrones on HBO Now and whenever I play a new instance of it, the monitor will consistently go black for a few seconds.

My drivers are up to date (version 9.18.13.5306). I'm not incredibly knowledgeable about computers, so I'm not sure what other information I should provide. Would really appreciate any help, thanks!

EDIT: For what it's worth, I changed HDMI cables to make sure the cable isn't faulty. Still the same issue. Also my second monitor (connected by DVI) seems to be fine.
