Feel like Computer is absolutely dying on me..

I got this computer in the Summer of 2013, and it has been acting up lately. I've recently been getting BSOD'd from a "bad pool header" and the ports are definitely starting to give out. If it's not too much to save it, I'd like to and then upgrade my graphics card.

To go more in depth about the issues I am having..

1. BSOD "Bad Pool Header": This only just started happening a few days ago. The difference from then and now are quite a few, but the significant changes I made from before the BSOD started happening was that I installed Malwarebytes & ESET NOD32 (I subsequently uninstalled ESET, but not Malwarebytes- also I have had CCleaner on my computer for a long while, and have been using it semi-often). At first, it only happened when I opened a certain game: Warframe. I had two installations of it, and when it came time to patch the game and update it, it would crash. I reinstalled the entire game and that fixed it. But then, it started happening without any game. It was random. I was watching a movie on my computer, with a few windows open in the background and then poof. BSOD. I was updating another game, with windows in the background and then poof. BSOD. I just don't know what's causing it. I also did a Chkdsk earlier today, lasted some odd 6 hours and there was nothing wrong. I also checked the optimization for my C-drive, and it needed 0% optimization.

2. Ports dying: This has been happening on and off again. I originally encountered the issue a LONG time ago, but it stopped and has only appears every few months or so. But, it happens. - Those top 3 ports there. However, it has happened to the back ports as well, but very, very rarely. Also, by "ports dying" I mean that the ports stop working (I will get disconnect notices and everything)... eventually though, they will "come back to life."

3. As for the graphics card, the fan is hanging on for dear life, and I just need to replace it. I don't need more powerful, if there is anything on par I am fine with that... preferably though, I'd like another AMD card. (I think there is a 500 or 550 watt ATX power supply).

1. Yes, I saw a few of the solutions for bad pool headers but they were so far a part and many. Some say that it could be certain programs, some say it could be RAM, and on and on. I figure I'll try deleting a few problem programs & files, and if that doesn't work I'll move to a system restore, and if that doesn't work I'll just refresh/factory reset my computer.

2. Most of the time it is just the USB ports. The main enemy are those 3 USB ports in the front.

3. What exactly should I look for in terms of size? Should I just give you a picture? Also, there's no sticker on my power supply so I don't know all the specifics. I believe it is a standard 500 watt atx power supply.

2. Check in Device Manager whether your usb ports are working properly and the drivers are up to date. Double check the case usb leads are plugged properly into the motherboard.

3. You'll need to measure in your case to see how long a video card can be. If its an included PSU then it won't be of good quality. Once you've worked out what card can fit then buy a PSU accordingly.
