Should I be worried about my SSD?

I have a Asus Z97-AR, i5-4690K, 8GB Crucial Ballistic Sport, Windows 7 Pro.
120GB Patriot Pyro C: (3 years), 512GB Crucial MX100 D:, 2TB WD green E:
Swap file is on the E:

Crystal Disk info reports fine (98%) and SSD Life says it will last until 2021 = ridiculous nonsense result.

I use System Mechanic and it keeps finding errors on the Patriot drive.
When it runs CHKDSK on boot, it finds lots of damaged files (usually Nvidia drivers) and 2 reparse records.
Could it be that I leave the PC on 24/7 and let it go to sleep?
Is the drive about to fail?
