GeForce GTS 250 Problem while playing games

Recently while playing games (CS:GO, League of Legends) my monitor turns off but I can hear sounds (music etc.). I have to turn off and then turn on my computer (restart does not work). I thought it was my monitor (Benq XL2411T) but after running GPU-Z test I think it may be graphic card (Gigabyte GeForce GTS 250 1GB).

GPU-Z Sensor log shows that in last 2 seconds GPU Core Clock increased from 738 to 740 MHz, GPU Memory Clock increased from 999 to 1000 MHz, GPU Shader Clock increased from 1836 to 1850 MHz, GPU Temperature increased to 255 and Fan Speed to 100%. I assume that the temperature is fake and my graphic card is turning off because of it but I have no idea why my Core/Memory/Shader Clocks has changed and what does it mean. I also checked temperature on my graphic card and max value was 66 while playing games.

My PC: GTS 250, i5 3550, 8GB Kingston RAM, Gigabyte Z77X-D3H, OCZ 650W
