Solution
<Set.SinceLaunchEvent> Graphics device rebooted
<Set.SinceLaunchEvent> Graphics device reboot queued due to d3d failure

Your graphics card f'd up somehow.
Either your graphics driver crashed, or your card got sent a piece of code it couldn't computer and thus failed. That's the sign of a bad/dying graphics card.

I'm getting a similar error today with the same game. Everything was fine until today, HOTS did an update as did my graphics card. Not sure if the problem is one or both.
