Okay so, since last night I'm facing a problem with my PC.

CPU: i5 2500k
Mobo: ASUS P8Z68 V Pro
RAM: GSkill Ripjaws 4 GB x 2 1600 MHz
GPU: Zotac GTX 960 Amp! (2 months old)
USB keyboard mouse
PSU: Corsair TX850 V2
Windows 7 Ultimate Edition

No overclocks

Everything's working fine till I find that my USB devices have lost power. This includes my keyboard, mouse, USB audio card and XBox 360 controller. Sometimes these devices are totally turned off, i.e. lights off. Sometimes one or two devices have their LEDs on but they are not responding. Disconnecting and reconnecting devices turns their LEDs off.

Moreover, the video card sends signal to the monitor but the screen freezes when the above happens. One time the PC was being shutdown and Windows was installing updates. The PC froze mid update. The only solution is to hard reset the PC. This freeze may happen after 30 minutes, or even a minute after startup.

One last detail is there was lightning in my area last night.

Memtest86+ booting from an USB thumb

Hdd with manufacturer tool

Test USB devices on other PC/laptop

Bios reset / clear cmos

Thanks. Will do. Another incidence I want to mention here is that only this one time the computer turned itself off and started back on without having to power it on myself. But it never happened again till now. Also the PC could freeze within minutes of startup even when idling, so not sure if I'll have enough time to actually run Furmark and Prime 95. Prime 95 should I run with small FFTs?

Okay, so I basically did a couple of things. Firstly I unplugged the PSU power and held the power button for about 30 seconds. Then I cleared CMOS through the Mobo jumper and reseated both RAM sticks, one of them in a different slot from the one it was already occupying. Then I disconnected my XBox 360 controller and USB sound card, keeping only my USB keyboard and mouse connected.

Oh, previously I had updated Adobe Flash Player as I read it was crashing recently. Then I booted in Clean mode. That is went into msconfig.exe. Chose to load only MS services on startup etc. Now running Prime95 small FFTs for an hour. As I write this it's been 4 minutes past an hour since I started the test. Temps are fine. No freezing nothing.

Before I forget, previously I was also running my mouse on the front USB panel. Now switched it to back panel.

Can I rule out hardware failure at this point?

Update: Ran Furmark for 20 minutes. Still in Clean Boot though. System's been running for 1.5 hours since last startup. No issues till now.

Update 2: Normal boot was fine for about 20-30 mins. Went to have dinner. Came back to frozen PC.

I will test those today. Should I run Memtest, one stick at a time?

Memtest threw no errors. HD test shows healthy. No errors. Was trying a system restore when my PC unexpectedly restarted itself and showed me the Windows did not shutdown properly error on startup. Dunno whether PSU or Mobo. Gonna install Ubuntu now and see what happens.

Thanks. But it still doesn't tell me what went wrong. :)

Also if Ubuntu runs fine, is it safe to rule out hardware fault, also keeping in mind that Memtest, HDD tests, Prime95, Furmark all came out clean?

Update: Also it seems my front USB was not really faulty. I'm running Ubuntu off a stick plugged into the front USB for a couple of hours now. I'm typing this on Ubuntu.

Update 2: Okay, so ever since the System restore PC's been fine. Only thing I remember happened between this restore date (14.07.15) and the date PC started freezing was Windows Updates. So I put Windows Updates on hold. Another funny thing that happened is that my PC suddenly started recognizing Bluetooth Adapters and began installing them. This one time I saw GPU temps rising and GPU loading even on idle. Rolled back drivers and it's fine.

Update 3: OK PC froze again after running well for 3 hours. Who to blame mono or PSU?

Indeed, keyboard could be a problem. Once, today everything was connected and post said, no keyboard detected. I've disconnected everything and letting PC idle. Trying to see when it'll freeze.

Update: Been running PC for 3 hours now. No freeze yet. Changed the slot GPU was in. Running only mouse for now. Typing this on on-screen KB.

Update 2: Reinstalled Windows. Installed only essential software. Everything was fine for 5-6 hours and then BAM! same old freeze. At my wits end now.

Not the same, but I am having the exact same issue.

I have
Asus 970 Pro Gaming/Aura motherboard
FX 8370 CPU
Asus RX 480 8GB OC Strix
2 HDD 500GB One Toshiba and one Seagate
PSU is Corsair VS650 Series
Corsair 8GB Vengeance ram 2x4 modules
Razer Abyssus 2014 gaming mouse
Wi-Fi adapter
Thrustmaster Ferrari Challenge limited edition Gamepad
and an acer keyboard.

The devices connected to USB 2.0 get disconnected and the sound device crashes and sometimes my video device also, my usb 3 doesnt crash. also after any crash I get a blue screen CRITICAL_PROCESS_DIED or UNEXPECTED_STORE_EXCEPTION.

I installed windows 2 times, fresh install in both HDD, because I though it was my Hard Disk. Bios is on latest version. I am also thinking that it might be the PSU or Motherboard. I also have to say that the motherboard is new also the CPU and the GPU, the PSU I had it bought last year and I had no problems with my older setup, so I give more weight to my motherboard.

drivers all installed yes, that about the 8pin atx12v i didn't understand it, what is the 4pins? But if I understand correct, I have connected 2 cables over the motherboard the one is the large rail and a smaller rail with 8pins, I guess you mean that.
And to explain further what my computer does, when I use it my usb devices that are connected to USB 2.0 stop working and i get the malfunction windows error repeatedly, after that my sound device fails (the onboard) and sometimes the sound gets distorted. The usb devices connected to USB 3 do not stop working. But after a while my computer completely freezes and gives me a blue screen with code CRITICAL_PROCESS_DIED

temperatures are very low 10-12 at idle, 35-40 under heavy load. I'll try to CMOS reset.
I found the instructions in the manual and succesfully did the CMOS reset.
I will use the computer for the next hours and I will write another answer here with what happens.
