This can be a bit long, but it's to give enough information because the problem seems to be really weird.
I'm using my old but yet serving very well GTX 460 1gb.
I was using 320.49 driver and windows 7 64bit for very very long time( about 2 years).
Was no issues so far, games working very well.
Some weeks ago i faced an issue with game called Insurgency from Steam.
The issue:
After playing a while, screen flashes to black, GPU fans stop working, and after a second the GPU starts to work again but you can't go back to the game(it's working but you can't see it, while windows works fine). The only way to leave the game is to end it process. Obviously i checked the GPU temps during this game, and they were fine.
70C at most with fans working on 70% -+
I thought it's game bug since it still gets new updates.
I switched playing Dota 2 and it worked fine for weeks untill 3days ago i got the same issue(screen flash black) and i understood it wasn't first game bug.
Then i tried another games and it happened everywhere after 10mins of playing..
I did fresh reinstall to my drivers(deleted current ones and installed again), didn't help.
Installed different versions, didn't help.
I blamed my windows, 2 years no formating, probably time to format.
I also bought air comperessed bottles to clean dust, and yes i had some alot of dust in the GPU.
I formated my pc and thought the problem is gone.
Nope, the temps are much better now but games will still crash, even on 55C.
Note: Games work very well, no lagg problems, just crash.
I got information of the crash from system reliability of windows, and it's same error before formating and now aswell.

Probably it's an actuall hardware problem, but what is weird that it works just fine.
I'm still hoping its some driver issue as it seems to be so.

Ok looks like my GPU is dead.
When i boot up my pc it crashes on the windows loading ( the welcome screen), black screen flash, then BSOD and says quickly something with "display driver didnt respond in time"
It all looks like overheating problem but the monitoring programs show fine temps.
Could be that my sensors are bugged and the programs show false temperature?
It could also be that my thermal paste got dry and i need to place new paste?
Right now i am using my computer with internal graphics card ;/
