USB Ports Stopped Working Windows 8.1

For no apparent reason my USB Ports (3.0 and 2.0) have all stopped working. I do not know exactly when they failed.

In Device Manager I have only one error under Other Devices that states Universal Series Bus (USB) Controller, this cannot find a driver to load.

I have an ASUS B85M-G motherboard and I have tried Intel's Driver check but to no avail.

Is this likely to be hardware or software?

Will updating to Windows 10 improve things or not?

I have still not managed to get this to work. Every driver download does not work.

The USB ports work in BIOS mode but not when Windows 8 loads.

In Device Manager I have:

Other Devices --- Universal Serial Bus (USB) Controller

This has the yellow exclamation mark against it and not searching for a driver helps.

Why would I just lose a driver?
