Yes your system can run GTA V on medium to low settings at 1080p with decent (above 30FPS) frames.
The minimum needed is:
CPU: Intel Core 2 Quad CPU Q6600 @ 2.40GHz (Your i5-2320, although old is definitively better)
GPU: GeForce 9800 GT 1GB (your GTX 750 Ti is up to three times better (3x) than the GT 9800)
RAM: 4GB (you have 6GB)
OS: 64 Bit until Vista (as long as yours is a 64-bit version of the OS, probably a Windows 7, you can run it)
