Something like a gtx750ti would be your best bet. Anything more powerful and you will have a CPU bottleneck.

That PSU is trash and needs to be replaced ASAP. This is the single most important part of the computer. Junk PSUs will easily damage or destroy the PC.
I would not use an AMD video card. They need more power and would be risky on that awful PSU. Get a gtx750 or gtx750ti (without 6-pin power connector)

Yes, that is a very good PSU, but you should get a 500w unit, so you can spend more of the GPU.

The 250x is slightly worse than the 750. It is your call really. Both will run new modern games at low/medium settings at 1080p smoothly.
Both will easily run old games like CS:GO at high settings.
