Is my graphics card going bad?

Hello,
I have a AMD radeon HD 7750. I have updated to windows 10 on the day of release and it has been working perfectly fine until today. I was playing a video game up until my monitor fell asleep and my PC restarted out of nowhere. Every time after that when I open a browser or a game my computer restarts and sometimes I get a windows notification along the lines of "display driver has stopped responding and has recovered". I have disabled my GPU and even reinstalled the windows 10 driver for it, the problem still persists. I am not sure if this is due to a hardware error of windows 10 or my GPU is just getting old. I read an article on signs that your graphics card might be dying and my screen does not have any tearing, blurry textures and the fan doesn't seem to get loud or hot, it just freezes and restarts my computer. I have been running programs like ccleaner and advanced system care to try and fix it and they run just fine. Steam can open aswell with no problems, just internet browsers and video games (all I have tested).
Any assessment of this problem would be greatly appreciated, Thanks!

I had a similar problem with 2 different video cards on Windows 7,
and possibly was caused by a conflict with the graphics drivers and
my soundcard drivers. I was able to finally get it all stable by rolling
back to an older graphics driver.

This is my PSU, http://www.impactcomputers.com/d10-300n1a.html
I know it is not very good since my PC is an HP purchased at best buy. I installed my 7750 myself and it has been working good for 2 years.

should note that it is the stock hp psu
