I'd recommend you to try using a different SATA cable to connect the hard drive with, or even try with a different SATA port on the motherboard. Afterwards, attempt to initialize the HDD again. If the error persists, I'd strongly recommend you to go to your HDD manufacturer's website and download their diagnostic utility to test the drive with. It will help you determine the health and SMART status of the drive. The I/O error might be also indicating a faulty HDD, in which you'd need to contact the technical support of the manufacturer/reseller and ask for a replacement.
