Random Blue screens/USB disconnects?

Several months back my Computer started to Blue screen crash Randomly about every 3 days (i never shut my computer off) And crash reports are giving me NADA (Unknown module and such) 2 weeks ago i stick a second hardrive in now it crashes every other day and USB devices (Headset/Controllers) Will randomly Disconnect Then instantly Reconnect about every 30 mins. This is Likely a failing PSU right?

Latest crash report Gave me this

So is this a driver issue?

I dont shut my computer off because i dont sleep well so im on it randomly throughout the night also i run a Plex server off it
