Need a new graphics card for AMD Athlon II X2 220

Hi everyone, so these are the specs to my computer, yes its far from the best I'm fully aware of that, however I am not a budget and don't have money to upgrade the entire thing. So for the time being I would like a new graphics card something that won't bottleneck that much and that my computer can run effeciently.

Mainboard : Asus CM1730,CM1830
Chipset : AMD 780
Processor : AMD Athlon II X2 220 @ 2800MHz
Physical Memory : DDR3 240-Pin DIMM 4 GB
Video Card : ATI Radeon 3000 Graphics
Screen Resolution: 1600 x 900
PSU: HBA 088-zA1gt 350 (286. 4w) total watts

I'm looking to play the total war series more, battlefield 3/4. With my current psu and processor I don't expect to run everything on ultra. I would like to run things on high/medium. Like I said I do have a budget of around $200

1. I'd like to know what would be a good graphics card for my psu, and processor with as little chances of bottle necking as possible and being able to run battlefield 3/4 on high/medium. I'd prefer to just upgrade the graphics card for the time being since I am on a budget.

2. If my psu is too low for any decent graphics card to run those games on high/medium, what would be a relatively cheap and good psu and graphics card combination to use with my current processor.

Do you know of any other cards that my psu could handle?

Yes, but they are all less powerful than the GTX 750 Ti. Open your PC. Can you see the nameplate on the side of your PSU?
