Graphics card and weird issues?

Hi guys,

So last night my computer froze and after a hard reset, my main monitor (i have 2) got swapped around, there are weird lines and blocks(which kinda looks like weird pixels) appearing everywhere since startup, my computer can't change resolution and can't detect secondary monitor.

I had tried resetting windows, reinstalling NVidia drivers and updating windows but nothing works. I also can't install the latest update for NVidia driver as after they prompt me to reset it's like nothing's done, update for 13 Aug still there and I can't go to any other tabs but driver..

Other than the above-mentioned issues, the computer works fine. Please kindly offer any assistance as I have no idea what else to do. Thank you all very much.

windows 10 64bit, 4gb ram, GeForce GTX 560 Ti

Your GPU driver is not functioning. It could be a software issue, but your description makes it sound more like a hardware issue. GPUs can wear out especially if they get too hot for too long. They may continue to function as a basic display output but with some serious issues.
I can't tell for sure based on just your description, but I suspect that you will need to replace your GPU.
