Do I need to replace Graphics Card?

I have only just ran into an issue with my pc that has the following traits:

  • The PC starts as usual, can login fine.
    About 1-2 mins later the screen turns dark in colour and faint vertical lines cover the screen.
    When I turn or move the screen it does not change in any way.
    I have tried a different monitor and have also tried different cables (HDMI, DVI).
I have had this computer for about 1 and a half years and have not had this issue before.
From the looks of the system it seems to be completely frozen however the computer continues to run.
I have taken apart the system and made sure everything was secure and no cables were loose.
My specs are:

Power Supply - 500W Corsair CX-500v3 ATX Power Supply
Motherboard - Gigabyte GA-H77M-D3H MB, Intel H77 Socket 1155
CPU - Intel Quad Core i5 processor i5 3570K LGA1155 3.4GHz 6M
RAM - G.SKILL Ripjaws-X 8GB (2x4GB) F3-10666CL9D-8GBXL 1333
Hard Drive - Seagate BARRACUDA 1TB,7200RPM,SATA3 6GB/S,3.5",64MB
Grahics Card - Gigabyte HD 7770 OC GV-R777OC-1GD 1GB 128-bit GDDR5
Monitor - AOC e2350Sd 23.0" Full HD LED Monitor

Any help is very much appreciated, also, I would really like to avoid buying a new graphics card so any help would be appreciated.

corsair cx units are known to fail early, can you test with another psu?

also overheating can cause this issue, do you monitor cpu/gpu temps?

@maxalge would this be cause by tue PSU? I have had minor issues with the PSU before so this may be the case. Sadly i do not have access rlto another PSU at this time.
