Intermittent Graphics card problem or other?? New Build.

I have a problem with a new PC build. These are my specs:

Motherboard: Gigabyte Gaming 5 Z97MX
Power Supply: Corsair 600W
SSD: SanDisk SDSSDA-120G-G25 SSD 120 GB
CPU: Intel Core i5 4690K Processor (3.5 GHz, 6 MB Cache, LGA1150 Socket)
Case:Corsair CC-9011070-WW Carbide Series Air 240
Graphics Card: MSI Nvidia Gtx 960 Gaming Graphics Card 4G
Memory: HyperX FURY Series 8GB (2x 4GB) DDR3 1600MHz

The problem I have is when the graphics card is installed. Without the graphics card installed I have no problems with the machine other than its limited on board graphics capabilities.

When the graphics card is installed sometimes the PC wont start at all. The fans turn on and stay on. The lights come on and stay on. The screen remains blank. There is no output from the motherboard display outputs and no output from the graphics card display outputs. The computer does not even get to the BIOS splash screen.

If I reset the computer from this state it will often recover. The display output on the graphics card will work, it will move past the BIOS screen and will boot windows 7. However when the little windows jingle plays through the hdmi the audio is slow and distorted. If this happens and I try to play a game it will be very slow with bad audio and lots of lag.

If I reset or restart the computer from this state it will often recover but not always. The display output on the graphics card will work. Windows 7 boots. The audio is good. The system plays games very well. I think to myself great!

Then next time I turn everything on. I'm back to the same sequence trying to get it to run correctly.

I have tried the following:
Returned the graphics card and have since tried a new one with the same results.

Changed the graphics card slots. I am using the recommended one from the manual.

Clean installed the graphics card drivers.

Disabled onboard graphics while using the graphics card.

Removing the memory and swapping in different blocks

Checking the power connections to the components.

Inspecting how the motherboard is seated in the case, it all seems good but I'm not a expert.

I would really appreciate if someone with more experience could help me decide what I should look into next.

In my opinion it has to be something fairly simple I am missing if the system can on occasions work perfectly!? But I could be mistaken!
