Problems with my display/freezing/crashing- Please help!

Display kernels keep failing periodically and i'm quick to blame my GPU. Recently its been fully crashing and I managed to save the most recent error:

I want to order a replacement graphics card but I'm not sure if that's the issue. This was my first computer build so I'm fairly new to this. Please help!

Ill check on the memory speed when I get back from work I'm fairly certain its at the stock 1600mhz. It has frozen/crashed from playing: starcraft 2, The witcher 2, The witcher 3 and League of legends. Sometimes it will recover but often its a black screen or straight reboot.
