BSOD (nvlddmkm.sys) with strips on startup

System Specifications
Windows 7, 64bit, 8GB RAM, GeForce GTX 560 graphics card.

In the beginning, a BSOD will appear when playing Heros of the Storm.
Streaks and rectangles would appear on the screen before the screen froze and gave the blue screen. I was able to restart the machine successfully (load to desktop). Surfing the net does not seem to trigger a BSOD.

I upgrade my graphics card driver via Device Manager->Update Driver Software but the same thing as described above happens. Surfing the net does not seem to trigger a BSOD.

However today was different after a BSOD while playing Heros of the Storm. The machine would load windows and then trigger a BSOD. No desktop screen. Computer runs fine in safe mode. I can also start windows normally if I disable my graphics card.

These are the steps I've taken thus far:

1) Install Geforce Experience, then use it to install the latest graphics card driver. (failed)

2) Downloaded the latest graphics card driver and installed manually (failed)

3) Uninstall graphics card driver via Device Manager->Uninstall, then use Geforce Experience to install the latest graphics card (failed)

4) Uninstall graphics card driver via Device Manager->Uninstall, then install the latest graphics card driver manually (failed)

5) Replace nvlddmkm.sys with a new one (failed)

6) Uninstall graphics card driver via Device Manager->Uninstall, then install the second latest graphics card driver manually (failed)

7) Tried windows start up launch repair/ system restore (failed)

What else can I try? Is this a software or a hardware problem? Note that all BSOD I encountered contains these strips. I only see strips during BSOD and no where else. My computer works ok in safe mode or when I start windows normally with the card disabled.

Download driver sweeper after uninstalling the graphics driver. Had a similar issue this week, could barely login to windows without getting the same bugcheck as yours. Simple uninstall didn't remove driver leftovers. Make sure to remove anything NVIDIA related with driver sweeper, including physx software.
