Overclocking fx-4100 failed

Hello everyone, I am new to overclocking and to this forum :)
I am trying to overclock my fx-4100 but I failed. My cpu is an Amd fx-4100 and my motherboard is an Asrock 970 extreme3. Here is what I changed at the bios:

Disable Cool n Quiet, C1E, C6, Turbo Core Technology
CPU Multiplier: [strike]18,0x[/strike] -> 19,5x
Vcore: [strike]1,4125[/strike] -> 1,4250
HT ref clock (which I think is the bus speed): [strike]200MHz[/strike] -> 215MHz

So, now it runs at 4.19GHz. I ran prime95 and after 8 hours, core #4 failed with this error:

FATAL ERROR: Rounding was 0.5, expected less than 0.4
Hardware failure detected, consult stress.txt file.

Probably I should increase the vcore, but I am not sure. What should I do to fix this?

First, why did you increase the baseclk? AFAIK, this is an unlocked chip so you could crank up the multiplier as far as it goes without touching the base clock at all. That saves you a lot of trouble with adjusting HT and memory multipliers.

So try 200*21Mhz at the same voltage and if that doesn't work try 1.44Vcore.

Well, I tried to increase only the cpu multiplier at 21.5x (4.300MHz) and the vcore at 1.5v but I get the same error. Then I tried a small FFT test and I had 0 errors. Memtest86 also gives me 0 errors.

1) I use 2 x 2GB ram sticks
2) Cheap 550W psu (gonna change it soon)

Is it possible that one of the above, can give me the error?
