GPU not detected or showing display after previously working - faulty? EVGA GTX 980 6+6 pin

I think I have a faulty graphics card. It has suddenly stopped working for
no discernible reason. I'm also scared that I have somehow broken it by not connecting it to the PSU properly.

I've tried a few configurations to fix this and really need your thoughts or
some help. Everything was working perfectly for a few days after building
my computer.Problem first started when I restarted the computer, 'no input',
then screens would go to sleep. It won't show me the BIOS at start-up.

I initially tried these things:

1. Test the RAM one by one in each slot. No display.
2. Test the GPU in different PCI-E slots. No display.
3. Updated BIOS. No change.
4. Checked UEFI set-up. PCI-E set as primary. No change.
5. Checked GPU connections and power. No change.
6. Checked PCI-E slots for damage. Nothing superficial.
7. Checked DIMM slots for damage. Nothing superficial.
8. Screens work when plugged into on-board display ports.
9. Tried a working GPU in motherboard. Displays through GPU.

At this stage all that I can do is use the on-board display or the working
GPU I have borrowed. The computer won't seem to acknowledge that the
hardware is present. I haven't been able to try the GPU in another system
but it appears it is faulty since another GPU works on my system.

The most I did before restarting the computer is to perform windows updates,
and change some of my power settings to tell the computer when to go to
sleep.

Is there something really simple that I am missing?

How I connected the GPU and PSU:

I have an EVGA GTX 980 with 6+6 pin and an EVGA G2 750W. The card came with two 6 pin to molex adapters. I can plug these in and use a PERIF cable that has four molex splitters and a 6 pin to connect into the PSU. My PSU has two 6+2 to 8 pin VGA connectors as well.

Do I use the two VGA connectors or the PERIF one to connect the molex cables together?

Of note are the adapters that came with the GPU. The 6 pin connectors that go into the GPU look different, they have a middle pin in the top row that is missing. The 6+2 pin VGA ones have one filled in. Also the VGA cable go into two separate 8 pin holes in the PSU (12 holes into 16 holes). My PERIF cable goes to a single 6 hole slot.

Does this difference matter? I connected the VGA cables because my GPU booklet instructed me to. Now I'm unsure and think this ruined the card.

It may be a faulty gpu with its display ports. You can either RMA it for a replacement or refund it for a different card.
For the power connections for a gpu, you should not ever use molex adapters. You should always use the pcie cables from your psu.
As I mentioned in a previous thread, using EVGA pcie cables is correct and fine. It is very unlikely the psu or the use of EVGA cables has "damaged" the card.
