Port Forwarding HELP!!

Hey guys, I used to be able to port forward then I don't know what happened but now whenever I port forward I go to canyouseeme.org and it says connection failed. I have a modem/router connected to a switch which my router is connected to. Please help!

Could you clarify the setup? Is it modem/router --> switch --> 2nd router?

Make and model of devices will help as well.
