How to convert raw to ntfs with USB flash drive?

I have tried the suggested way but my drive still says in order to use format this USB flash drive. But it will not format from win xp or win 7. Nothing is working any help?.

@Phillip Corcoran is right. You might be dealing with a faulty flash drive. The RAW format usually indicates a corrupted file system. If you had important data on it, I'd recommend you to turn to a professional data recovery company.
I'd suggest you to try to format it from Disk Management or DiskPart (command line).
