Problems after installing new graphics card.

AMD FX6300
8 gb ram
Corsair VS550
MSI 970a-g43
Display: LG Flatron VGA Monitor
OS: Win 10

I recently replaced my Galaxy GeForce GT630 with Asus Nvidia GTX960 2gb Strix and since then my PC has become bit slower. I observed that the Disk section in task manager was remaining in 95%-100% . I fixed that by googling and disabling some processes. But when I start to play games the disk section goes to 100%. Previously on GT630 I played CoD AW on very good settings like acceptable settings but now on GTX960 its like lagging a lot!. Every game with graphics required lags I reverted to win 8.1 to face the same. These all problems started after installing the GTX960.

I ve tried installing drivers correctly. Any solution or thoughts are welcomed. Please help.

Thanx you for your replies. @PcBuilderProbs I somehow watching the HOWTO videos managed to reduce the 100% disk usage but it again increases during any gameplay. @evyosh I will surely follow your advice about defragger but this wasnt the case when I had GT630. Even CoD AW was running lagless. Is it the PSU? Coz GTX960 requires an additional power through 6pin connector. When I checked the system using HardwareMonitor I got to know that instead of getting +5V my pc is getting 2.9V and the +12V rail is providing 13.4V . Please help me wid this.
