Graphics card may be dying

Hello, and nice to meet you all. My name is Jonathan and I am having problems with my card.
I have a power color AMD HD 7870 Radeon most edition. My mother board is a MSI Z77A-G41 and my tower is a Thor V2 and I have a Kingwin 1000w.

I need help finding out if my card is dying or if it can be fixed
A little background on it

It has started to heat up to 75+ degrees and the fan spins like crazy and on the other hand when I turn my pc on the cards fan doesn't work only when it starts to overheat. I just recently noticed this a couple of months ago.
In the BIOS of MSI the card reads very low voltage.
The Frame rate has gone down. I use to be able to play Tomb Raider at 50-60 fps. Now it only goes between 20-35.

I also updated to windows 10 idk if that could be part of it.

If you need any more info ask me. I just need help please, I just want to know if it's something I can fix or has to replaced.

All drivers are up to date

Well, then you can try search for older driver manually on the internet.
Try different driver until you find the most stable driver for your card...
