Fairly new to buying graphics cards... But I've heard this one is good but what games can it run?

The 380 and GTX 960 would be your best choices then, I'd personally go for the 380. What are your full specs though, so we can check your system can run it.

Your CPU will bottleneck your GPU in certain games, unfortunately.

As for your games, GTA 5 should work fine on your current card if you drop AA. (Your CPU may be the issue here)
Fallout 4, you won't be able to turn the settings up very much, even with the new card.
Black Ops 3, you should be able to run that at fairly high settings on your current card.

The recommended PSU for the 380 is 500w, I'm not sure how the quality is on your one.

AA means Anti Aliasing, which smooths jagged edges in games, sorry I somehow didn't see that bit.
I would normally only recommend Intel CPU's but as you are on a limited budget, I'd suggest considering the FX 8350, you should be able to upgrade the CPU with your current motherboard, I'm not 100% sure as AMD CPU's have more than socket problems with motherboards.

£300 is a bit of a guess, because since I'll have to wait until Christmas time to get some money.
What's the best graphics card I can get with my current set up? Or is that kind of hard to guess?
 

In most games I believe your CPU will be bottlenecking even your current card, use MSI Afterburner to check your CPU and GPU usages while playing.
