AMD Radeon R9 won't detect third monitor

Have an AMD Radeon R9 R9 280X which so far has treated me very well...

EXCEPT...

Despite the fact that Catalyst detects a Panasonic HDTV (which doubles as both television and as a computer monitor), it will not display it as a 3rd monitor when plugged into the mini-DP to HDMI adapter. It does get picked up at boot, and the Windows load screen comes on it, but it disconnects once I get to the login screen.

Furthermore, it looks like the card will not display any more than two monitors at any given time. Connecting one monitor to the DVI-to-HDMI port will work, and the Panasonic HDMI straight to the HDMI port will work, but monitor two won't work at all.

However, my other two monitors will connect just fine, without issue, to the mini-DP to HDMI adapter.

Confirmed all driver updates are applied, but I'm scratching my head in terms of what it could be.
 
so if you were to plug in all three displays except instead of using the DP adapter on the TV use the adapter on one of the monitors does the monitor now not work or is it still the TV? This will clarify if it's an issue with the cable or the TV. You no longer need to have a DP in the setup of triple displays, so you can use a DVI-HDMI cable or other alternatives to test.
