GTX 750ti spins up to 100% fan speed, but outputs no video.

Back in June, 2014, I bought an EVGA GTX 750ti SC edition. It's been working perfectly, up until the last month. I turn on my computer one day, and the fans on the card spin up to 100% speed, but the card outputs no video. My onboard graphics do though, and I see that my card is not being recognized at all. Fast forward about 5 days, and I decided to reconnect it to test it before I send it off, and it functions perfectly.

This lasts for about 3 weeks, before, today, the same problem arises. I've updated my motherboard BIOS already, and the problem persists. Just 5 days after Fallout 4, too. Any suggestions?
