Presently i have radeon hd 5450 2gb ddr3 but i am not able to play witcher3 due to reason of graphics card stop working

The Radeon R7 260x is much better than all of the previously listed GPUs. It won't run max settings on those games with anywhere close to 60fps though. The most you can probably run those games is medium. May I suggest a 750ti? Its more powerful than all of those listed and you can pick one up for around 100$, sometimes less, sometimes more.
I have an MSI 750ti OC, it works really well in current games and paired with a decent-good CPU is your best choice. I picked it up for 95 bucks through PCPartPicker

+1 on GTX 750 Ti. I had one myself, until I upgraded. Depending on your other hardware, you should be able to play most games with normal settings at a rate near to 60 fps at 1080p. For newer and higher demanding games like you mentioned (Witcher 3, Black Ops 3) you can expect a little more than 30 fps at normal settings. That is at console level.

GTA V would have 52 fps at high settings

I agree with Sam_missles. If you can spare the money get one of these two cards. Although I would recommend the STRIX but it's about $30 more.

ASUS Geforce GTX 750 Ti about $130 after tax and about $100 after rebate
ASUS Geforce GTX 750 Ti STRIX Edition about $150~$160 after tax (no rebate available)

The standard one will save you a good chunk of money with the mail in rebate.
 


You need to list your full system specs, including CPU, RAM, case model/brand, power supply mode and brand to get proper advice on what video card you can use. Buying a video card that won't power up or fit in your case is not a good idea.
