100% disk utilization with 0 kbps read/write

My disk is constantly sitting at 100% disk utilization, despite no read/write processes occuring. I tried scanning the drive for errors, but I was told it was error-free and that I shouldn't bother with scanning it. I tried doing this, https://support.microsoft.com/en-us/kb/3083595 but it wasn't of any use. I ran adwcleaner and removed everything. I've disabled superfetch, windows search and BITS.

I'm really at my wit's end. I'd replace the drive but I honestly don't know if it'll solve the problem.

I'd be happy to provide any reports based on diagnostic tools. I have crystaldiskinfo and process monitor downloaded but I wasn't sure what to make of the information provided.

This seems very unfortunate, Sanjeev, I'm sorry to hear about your WD drive! :( I'd strongly recommend you to backup any important data from it and copy it somewhere else ASAP. Then, I'd suggest you to check the warranty on the HDD here: http://products.wdc.com/support/kb.ashx?id=FhgXBX
If it's still covered, you should get in touch with our customer support and send an RMA request, in order to get a replacement.

Great job with the troubleshooting so far! However, I'd recommend you to run your HDD manufacturer's brand-specific diagnostic tool as well and benchmark the results from the other testing utilities as well.
Another thing that you should try is swapping the SATA cable that is connected the drive as well as changing the SATA port.
I'd also go to your motherboard manufacturer's website and check for any SATA controller driver updates (or even try reinstalling them).
While on it, you might also want to search for any BIOS firmware updates and make sure you have the latest version.
Hopefully, one of these will resolve the problem. If not, I'd recommend you to use another computer to check up on the HDD and what its behavior will be in the other system.

Thanks for responding SuperSoph. chkdsk says the drive is fine, as does HD Sentinel. However, the drive failed both the quick and extended tests for WD's diagnostic tool. I am unsure what that means. The sata controller seems up to date. I updated the BIOS. I'm not familiar with how to change the SATA cable or port. I don't have another computer to test the drive on.

Update: I installed new AHCI drivers from Intel's website. I also reset my system keeping my personal files using Windows 10's reset option. No dice.
