Possible Hardware Issues Causing Blue Screen

Hello Everyone,
So, I built my computer about 6 months ago to run some games. For about the first two months I was having a lock-up issue with it, however I found that issue in the factory preset BIOS and fixed it. About 3 months ago, a new issue arose. Now my computer's USB connect ANYTHING will turn off (completely unresponsive) and the audio will generally go crazy (either robotic or cut out totally) before or concurrent to the USB lockout. After about a minute of my computer just sitting; it will Blue Screen and say Critical_Process_Died. So, I've had this issue through clean installs of Win8.1 & Win10. I am led to believe it is a hardware issue. Yet, I have added in a new HDD and recently replaced my GT 610 with a GTX 960 GAMING 4G . I have tried running bare minimum USB devices. Something note worthy may be that once in awhile when this error occurs I can unplug devices then insert them in a different spot to regain control, however the audio is broken during this time. This is probably some simple issue that I am overlooking, but as of now I have run out of options that I know of. So, I'm asking for some help, thanks! :)

Well, I would do that, but I need some sort of evidence to replace my PSU, I have had it in since the original build and it has never been problematic. I have taken out the 2 HDD lower storage to see if maybe the power to them was causing an issue, but it didn't effect it. Anyway, do you know any solid way to test the PSU?

Hey again, I ordered a new EVGA SuperNOVA 850 B2 , installed it and checked through the wiring just to be sure. Unfortunatly, after a day, I encountered the problem again.
