Code 43 in Device Manager - Nvidia GTX 960

I finally put together my first rig last night and was stocked it actually POST (had doubts).

Although after updating BIOS, installing OS, Windows updates, Driver updates, I come across my GPU getting the "Code 43" in the Device Manager.

In BIOS my resolution will be set to "normal" (I am unsure of dimensions, everything looks normal), but as soon as I boot into OS I am sent back to 800x600 and my card get that error. My CPU has an integrated GPU so I am assuming it is jumping to the onboard card when OS opens for some reason.

The PCI has been set to default and I disabled iGPU. I've tried uninstalling and reinstalling, multiple times. I even wiped Windows to try again, nothing. Code 43, everywhere. Please send help.

 
If you have clean uninstalled the driver and even the windows, then this can only mean a faulty GPU. Generally code 43 appears due to incorrect communication between the OS and gpu and is fixed with clean driver reinstall, but if that doesn't work, chances are most likely it is a faulty gpu.

Exactly what I did not want to hear.. hopefully I can get a new one if this is the case.
