XFX R9 390 very low preformance

Hello,
I just recently purchased an amd r9 390 by xfx to put in the PC I built. I am not sure why, but I am getting horrible performance from a card that should excel at 1080p. I am using it strictly for Fallout 4 at the moment (below is an image of the settings I am using), and I am getting FPS as low as 9!

I have the Crimson 15.12 drivers (the first ones I ever installed on my new system) which I believe are the latest. Nothing looks or sounds odd about it from it's exterior, my highest temps was a healthy 60 degrees Celsius. I have tried turning the fans to 100% - no change. Below is a screenshot of my MSI Afterburner measurements while in Diamond City in Fallout 4 (the most demanding scene).

The core clock seems very low.

The temperatures while running Fallout 4 are visible in the Afterburner image. They don't seem to go past 62 degrees Celsius in Fallout 4, but get up to 78 when running Unigine Heaven. The gpu usage is everywhere good point. What about the core clock, it never passes 750 mhz?

Speaking of Heaven I just re-benchmarked, here are the scores (thought they might help):

I have 8GB of DDR4-2400 RAM, 8 GDDR5 in 390

While on high settings
CPU usage: up to 98% (only once), avg=60 according to MSI Afterburner, up to 75, avg=40 according to task manager (this is quite a bit more than a few days ago (max 40))
CPU temps: 30-40 degrees Celsius.

On the lowest (absolute lowest) 1080p settings, I was still unable to handle a solid 60 fps, sometimes dipping down to low 20s (WTF?!).
The usage continued to drastically spike between about 9 and 100 percent usage. The core clock was never greater than about 750 MHz. See MSI hardware monitor screenshots below. The CPU went up to 100 usage once! Also, could someone please clarify what the page file is and if the stats are ok (8000 Mb/s)?

One thing that I notice is that you said you have the Crimson 15.12 drivers, but MSI Afterburner says you have Catalyst 15.8. Either MSI Afterburner can't detect Crimson, or your driver installation went wrong. Try installing an older driver version after removing the current one. Maybe from the disc that came in the box, if it came with it...

If that doesn't work, well, we have to start thinking in the direction of a faulty driver, if we assume everything was connected correctly. Double check your power cables to the graphics card.

Sorry I took so long to reply, I have checked my drivers thoroughly. Initially I thought it might be a driver issue because they never installed correctly, but I used DDU (Display Driver Uninstaller), completely deleted my old drivers, and installed Catalyst 15.11.1 beta (15.12 still had the Fallout 4 issue for some reason) correctly (my antivirus was the issue). MSI Afterburner now displays 15.11. Also, I checked my power cables and I believe they are installed correctly [6+(6+2)]. I have until Wednesday to submit an RMA, so if I cannot figure this out, I will just do that. Thanks again for your help.
