Want to upgrade my PC, but where do I start?

I'm looking to upgrade my PC, but I know little to nothing about PC tech. I'm hoping someone can help guide me in the right direction. To be honest I don't even know what I should upgrade.
My specs right now are:

AMD fx 6300 (6 cores)
Geforce GT 640
64 bit OS
My motherboard has AM3+ written on it

I've looked into the GTX 900 and late 700 series, and they seem good enough, but are they worth it?

EDIT: Checked out some more specs, looks like I'm down between the GTX 960 and 750 Ti

Go with a 960 with 4GB of VRAM and consider a processor upgrade, to the FX-8320 or something. That should put you right in the middle of the 300 range and you won't bottleneck yourself with the 6300.

I'm looking at my switching power supply (it's the big box isn't it?), and it says a couple things
450W DC output
115V/230V~, 10/5A ,60/50Hz AC output
Is that it?

Well the price isn't much a problem, but I'd say 300-400$ (some of the products I've seen on the Geforce site are around 200 though :/ )
