Phone always hacked

Need info on how someone is able to access all of my info on Samsung galaxy s6(SM-G920R6 and monitor, change, read, all my info with out my permission?

FWIW my brother in law put an app onto my sisters phone that would let him log into a site on a desktop computer or mobile and he could see all kinds of data. all texts and calls, their time stamp, sender and receiver, and content was all made available. even gps location of both sender and receiver. but the app doesnt show up in the app drawer. maybe go into the play store app, open your "my apps" section and see if anything stands out that you don't recognize.
