Possible PSU issue

Hi,
Today I have experienced some weird issues I think may be psu related if they're not a one off kind of thing

Firstly my keyboard, a razer blackwidow chroma, became completely unresponsive but was still lit up, after about an hour of try to fix it varied from briefly working fine, to not working with lights on to not working with no lights on, I tried changing usb ports and found I got more reliable usage with the ports on the front of my pc (normally plugged intothe back) anyway uninstalled synapse and all drivers for it, found 2 ports in the back it works fine on and haven't had anything else yet in regard to that.

Secondly, I went to turn my pc off earlier and the screen went black (normal) but it stayed on for ages, I ended up holding down the power button and it switched off but my keyboard stayed lit, so I switched the rocker switch on my psu and it was still lit up, ended up having to unplug at the wall.

Thirdly, I plugged my phone into a usb port and it worked briefly but then I could not get it to charge or connect, tried different ports with no luck.

I know it's not as accurate as a proper hardware monitoring tool you plug in but took a screenshot from cpuid's hardware monitor with the voltages to see if anything is weird there, will attach it below.

Windows 10 64 bit
i7 4790k not overclocked
gigabyte g1 980 ti factory overclocked and stable
16gb of ram
Corsair ax1200i (I know it's overkill but had a bad 860i and could only get this on the same day)
