How do I fix a GPU fan that grinds against the bottom of the "heat sink" looking thing beneath it? (nVidia GeForce GTX 760)

Hey everyone. So here is some back story if this changes the solution you can give me in anyway.

2 days ago I was play some video games and randomly both of my monitors stopped getting a signal from my computer. I was baffled as to what had happened. Only solution for me was to manually shut the power off by holding the power button on the front panel of my PC. I open up my game again and after just a few minutes, same thing happens. This goes on for a few days. Randomly it will work and sometimes it just wont let me play...AT ALL. I then opened up the computer, and cleaned out the dust in it with some compressed air. This fixed the problem and I was able to play games again.
1 day ago. I was playing again, forgot completely about the whole spiel that happened the day before. Few minutes after loading up the game, same thing happens. Monitors lose connection and are given no signal from the computer. I solve it by shutting it down and rebooting the PC and it's magically fixed.
Today. I open up the PC again and I dig through it and detach the graphics card. And find that the fan that cools off the graphics card isn't spinning...AT ALL. I open up Open Hardware Monitor and my GPU temp. is at 120 degrees Fahrenheit and I open up MSI Afterburner and use the fan speed changer and nothing works. When I used MSI Afterburner, the fan would spin, but so inconsistently and made lots of "grinding" noises that I didn't trust so I just turned it off. I have 2 regular house fans blowing directly into the open computer (I have taken the panel off).
I now know the problem to the whole monitor thing and things that are going wrong. I would like to see if I can just repair it at home before I go to Best Buy to repair it, or buy a new card.

Please. If anybody has a solution, or even the slightest thought as to a solution for this, please drop it below. I would greatly appreciate that. Thank you everyone, and have a nice day.

Don't open up your GPU until you check your warranty information. The GTX 760 came out less than three years ago, so even if you bought it on release date, you may still be under warranty, depending on the manufacturer. If it's under warranty, your best option is to RMA it.

The fan sits on the GPU and spins on magnets of some sort. Which is weird.

That's not very specific. Which 760 do you have?

It's an ASUS nVidia Geforce GTX 760 (192-bit)

No, that is not the exact one that is in my PC right now. It has one fan in the middle of it. Primary color on it is red. But by the look of it, the specs look similar.

Call ASUS - there's a solid chance your GPU has a 3-year warranty.

Alright. Here is the GPUZ that you linked me to the download. https://gyazo.com/1adedaeb1297c3ff0813b0a322edcc40

Here is what the fan side of the GPU looks like: https://gyazo.com/a705938193a46f07a6f4b4247fb8b27f
Here is what is beneath the fan on the GPU: https://gyazo.com/f4e64afc2110aa740a0cd96a95d7a9e8
Here is the other side of the GPU: https://gyazo.com/c53517ff2f02ee2ce8191124f7c51ded
And here is the under side of the fan itself: https://gyazo.com/3e360e61487cd546d3e5efc1df06c5a7

Thanks.

It seems your fan should have ball bearings beneath the fan cover:

Your fan seems to be missing those.
They must have fallen off; if they're still in the case, you might be able to replace them.

Which one? AFAIK you won't find your exact card in the open market. If it's a normal GTX 760, it's better than the OEM one you had.

Bought an nVidia GeForce GTX 960. Yeah, I'm pretty sure something fell out. I couldn't find any thing on the bottom of the PC as if something fell out.

Yes. I was playing Bioshock Infinite on Ultra and got 90 frames throughout WHILE livestreaming. Very good card. Anyway, thanks for all the help. I greatly appreciate it.
