Do I need a better Graphics Card?

I'm trying to get my pc up to snuff so I can use it for gaming. I know all the hardware I need except for the graphics card. I have one already installed and in use, but I'm not sure whether or not I need to replace it. All the forums I've looked on say a gddr5 runs a lot better than a gddr3 but I'd rather not spend money on something I don't need to buy. The specific model of my graphics card is a Radeon HD 5450 and it's a GDDR3 with a 512 MB memory.

I'm sorry, I didn't see your specs. It might just be time to build a new PC...save up the money and bite the bullet. At least, that's what I would do. Those parts are really outdated.

Yeah specs would help and maybe a budget (if you have one) but for 1080p gaming high-mid graphics I would recommend gtx 970, gtx 960 or r9 390. for cpu really anything i5 would work. if you want a build I have a few that u may be interested in on pcpartpicker.

It's an older computer. Maybe five-ish years old. I'm just trying to replace parts that need replacing.
CPU: 2x Intel(R) Pentium(R) D CPU 2.80 GHz
Display: Running dual screens but biggest is 1920x1080
Power supply: 300 watt
Ram: 3 GiB (I plan on replacing/installing more Ram first)
Mem: 154 GB
Budget: $400
I'm aware I might just need to get a new computer but it's a nice case and if I don't have to get a new computer, I won't

You REALLY need to buy a new power supply. 300w is too little, and plus it's a very old one so a new 450-500w powersupply is important. Buy from a well known brand like Corsair, Seasonic, Silverstone, EVGA, Antec, Cooler Master.
Also the CPU is not very powerful, you might need to buy a new CPU and a new MOBO, since that's an old socket and there's no new chips that will work with that motherboard. Buying a new powerful GPU, while having an old CPU will cause a bottleneck.
