what could i have done?

I installed a dark theme a while ago and manually changed the text to the neon blue awhile ago because the default white is to bright for my eyes and gives me head aches. Everything was fine for awhile but the other day i noticed this glitch on my secondary drive, it's only on the secondary drive tho and appears to be only this file. It has not seemed to affect my C drive or other files.

I just hope the drive isn't failing, Any idea what might be causing this or how to fix it?

When viewing the secondary drive, on the top left of the windows panel click on "organize, now click on "folder and search options". Now "folder options" will open up, next click on "view" and click "show hidden files and folders" now look for any cache files left by the dark theme that may be still changing color of text on that secondary hard drive. I doubt your hd is failing.
