Monitor losing signal randomly when playing certain games

Title.

The screen loses signal but the audio keeps playing in the background. Doesn't even crash to desktop, whole PC gets locked up. From there, only option is pressing the reset button. Super annoying.

Specs: i5 4570 CPU / 8GB / R9 285 (Sapphire ITX Edition) / Windows 10 with latest crimson driver (16.1)

Max Payne 3 was pretty much unplayable (crashing/losing signal every run after about 15 minutes).
Fallout 4 - happens randomly.

The GPU is overclocked to 1040MHz, but I tried with stock clocks and it still encountered the issue.

Need your help on solving this

its an FSP Aurum series 500W Gold rated PSU that is barely an year old. The mobo is not ITX, just the GPU.
However one thing to note, I have no issues with games like Dota 2 or League of Legends or just general Windows stuff. The problem is only there when playing certain games. My guess's this is some sort of driver problem? I have done some googling and found out several other R9 285 and 380 (essentially the same GPU core) have had the same issue.
