missing network adapter in the device manager

my onboard ethernet is already dead. im using a pci netwok adapter for several years now, then out of nowhere it seems that my network adapter disappear in the device manager, my rig is 5years old already. what really is confusing here is that sometimes it comes back then disappears again the next day or after restarting. whenever i tries to troubleshoot it from the tray icon, it says "windows cannot find driver for your network adapter".

these are the few things i already did:
*installed driver
*no unknown device in the device manager
*cmos reset
*clean install of os
*unplug cpu from the main power then press on/off button several times to juiced out the remaining power
*used a usb type ethernet
*used a new pci ethernet

clean the slot of pci? i also used a usb type but it wasn't detected, the network adapter in the device manager is still missing.

is it already a hardware problem? mobo? software?

i know it's old, but still very usable..
