Mouse appears to be spazzing out after a quick swipe on the mouse pad surface, yet this does not happen on, for example, the table surface. Could this be some kind of a problem with the mouse pad's surface or the mouse's sensor?
Both would be the likely culprits. I've had a near similar issue when using the Microsoft X8 mouse on a Razer Goliathus Speed Edition Mouse pad and the cursor would go all over the place. Reduction of the mouse;s DPI settings took care of it but I ended getting a Steelseries QcK+ mouse pad and also regularly look at the bottom of mouse to see if the sensor managed to pick up some stray fibers which may cause sensor issues. So for me it was the mouse pad.
Rule out the issue with a borrowed mouse pad from a friend and see if the issue is evident.
