PC randomly shutting off while gaming

Okay so for the past month or so after a while of gaming my PC will sometimes shut off unexpectedly without any warning. Has happened several times while playing Witcher 3 this week.
After monitoring my temps my GPU highest it gets is around 68 C 99% load.
My CPU peaks at 73 C with 91% load.
This is really annoying me, I'd appreciate any help!! :)

SPECS
CPU Intel core i5-3570K (stock fan and heatsink)
GPU MSI GTX 970
MSI Z77A-G45 Thunderbolt MS-7751 motherboard
16 GB DDR3 memory
PSU Seasonic S12II 620W BRONZE

UPDATE:
I realized I did something really stupid... I guess must of mixed up my monitors 120v cord with my PSU's 300v cord. Switched them and now am going to do a test and see if it still crashes. Will update with results.

UPDATE 2: Well I played Witcher for about 2 hours and didn't have any issues so far. If it happens again I will be back with another update

UPDATE 3: Installed new CPU cooler and still happening

When a pc shuts off with no reason it is generally a sign that something is wrong with the psu so if it continues I would look at a new psu. Reply back if it happens again.

Thanks for the reply!
So it did happen again today unfortunately.
I even installed a new EVO 212 cooler for my cpu and it still happened. True I believe it may be a psu problem as well since I think everything else checks out. What PSU do you recommend? I would like something affordable that I can OC with and wouldn't have to update for a while.
