Samsung 840 Evo 256GB slow

I have a Samsung 840 evo and it is acting incredibly slow.
When trying to transfer fallout 4 from the ssd to the hard drive, the Hdd responds but the ssd the these;
Transfer speeds ranging from 8.82MB/s or lower to 300+MB/s
100% active time.
0ms response time
0kb/s read
0kb/s write
As well as extremely low random write iops.

The system also becomes unusable when trying to transfer the files.

I'd follow @SBMfromLA's advice and download the SSD brand-specific utility to check if everything is okay with it. It would also allow you to update its firmware if needed.
Another important thing you should check is your BIOS and make sure that the SSD is connected to a SATA III port with SATA mode running in AHCI. You can also refer to the motherboard manufacturer's website and see if you have the latest SATA/AHCI drivers installed for your mobo.
I guess it won't hurt to check the physical connections to the solid-state drive to determine that it's mounted properly.

How much Free Space do you have on your SSD? It's highly recommended to have 10-20% Free Space on your SSD to keep it running smoothly.

Also, since you have a Samsung SSD, you should download and install Samsung's Magician. It's their SSD Utility Program. With the Utility, you can OVERPROVISION your SSD, which will extend the life of it and also have the program OPTIMIZE your SSD and Computer for maximum performance.
