Phenom II 965 above maximum frequency

Hello, I noticed strange behaviour of my AMD Phenom II x4 965. It's overclocked to a stable 3,8GHz and everything was fine for a long time. Today I noticed (while playing GTAV), that the frequency is about 100MHz above 3,8GHZ (about 3,890GHz), which caused GTAV being unstable (without overvoltage 3,8 is max stable frequency of mu unit). I lowered the OC to 3,7 (during the GTAV it was at 3,8 again about 100MHz above my OC). This happens also with multiplier set to auto (at stock 3,4GHz) and again it's about 100MHz above stock (and the game was unstable in every case mentioned - for example mouse cursor was lagging as hell). This happens mostly during the GTAV but also during the system start and while most of my games starting. What may cause this strange behaviour?
AV didn't show anything, hijackthis also, mallwarebytes anti-mallware also.

MB: gigabyte ga990xa-ud3
PSU: OCZ ZS650W

EDIT: I noticed 2 things also. 1st is that sometimes fq is about 50mhz lower than 3,6GHz and 2nd is that the NB frequency jumps also (from 2020 to slightly above 2100mhz

I would say it must be the motherboard. Set the board's BIOS back to default. Use the reset if necessary. If it still happens at stock settings, I would suspect a problem with the board. If all is well, OC doing nothing more than raising the multi. See if the problem returns, but don't change anything but the multi. If it returns, I would suspect the board for sure.

I did that and it's happening again. I checked the frequency in 3 programs. OCCT and HWMonitor are showing this weird values while EasyTune6 (included with MB drivers) shows only about 30MHz. Also the BUS and HT(or NB, can't tell because ET6 doesn't tell) frequencies are going crazy (about 200 - 210 for bus and 2000 - 2100).

Of course, the only way to verify for sure if it is the board and not the CPU, would be to try the CPU in another board... or try another CPU in that board. Not so easy to do unless you have access to such. Do these spikes show up in BIOS too?

I have no acces to a different cpu or mobo (non of my friends/family have such a thing). My BIOS doesn't show actual frequency. It shows only what I set :(
