PC won't display on monitor with the video card plugged in

I have been using my self-built computer for about a year and suddenly the monitor is showing "there is no signal coming from the computer".

Below are the specs:
-Intel i5-4460
-ASUS H97-Plus
-2x4gb Crucial Baliistix
-EVGA GTX 750Ti SC
-Samsung 840 Evo 250gb
-WD Blue drive 300gb
-Win 7 Home premium

When I connect the DVI cable to the onboard graphics, it boots up fine and displays.
When connected to the card, it beeps once but then no display.

Not sure if I should RMA the graphics card or do clean install of Win7 or upgrade to Win10.

I tried clearing CMOS and disabling the onboard graphics but no dice.

I think I was installing some drivers from SlimDriver and the issue occur. I think the driver was called "PCIe Interface controller". I tried System Restore but did not help.

Now the PC is giving me "1 beep, 3 beeps" at the same time every day even though the system is off and I removed the system speaker.

My SM bus controller is showing "The drivers for this device are not installed. (Code 28)" and
"PCI bus 0, device 31, function 3"

When your PC is booting up... in POST screen... VGA drivers are not loaded yet!
Thats why there is no way that its a drivers problem!
If your PC was booting up normally and it was rebooted, or give you a BSoD, just before it shows you the Desktop... THEN its a drivers problem (probably), because that is the moment when OS is loading VGA drivers!

Now... its sounds more like a faulty VGA!

Can I check how to RMA the graphics card...do i need an anti-static bag?

You can contact your supplier and ask about the details...

If you have the original box with the antistatic bag, use it...
If not, I don't believe that it's going to be a problem!
Just make sure that it will be a good package with protective bubbles or something and shield it with some tape.
It's winter... And you don't want any kind of moisture in there, because there is always a chance that your supplier will see the moisture and refuse the warranty...

So now I have removed the graphics card from the rig and is using the Intel HD graphics,
now my system gives off the same series of beeps every day at the same time even if I removed the motherboard speaker.
The beeps goes "1 beep-3 beeps-multiple beeps"

The motherboard manual does not come with the beep codes; any idea what's causing the beeps?

Another issue: I have uninstalled the Nvidia drivers but they are still showing up as installed under Control Panel -> Programs & Features. However nothing happens when I click on 'Uninstall'.
The driver does not show up under Revo Uninstaller.
