[SOLVED] Screwed up my flash drive, tried reformatting, and it didn't work

I screwed up my flash drive and made it bootable, but I can't format it on DiskUtil. It won't let me erase data on it, or change the name. It keeps spitting back an error.

Can someone please help me?

Thx for reply but I solved problem. My MacBook Air is screwy with erasing for some reason. I just used a MacBook Pro and it worked fine.
