Horizontal lines on monitor while playing games

Every time I try to play a game with decent or better graphics I get flickering horizontal bands across my screen. I notice them most when I turn in-game. I hate to say it but I'm a bit out of my depths here and could use a hand.

Specs:

Intel i5-4670
Nividia GTX 760
16 GB RAM
 
First thoughts are bad video card, but could be heat issues, power supply issue, driver issue, even RAM at times. If anything is overclocked, set to stock speeds. Make sure the case and fans are clean, monitor temps with a utility like HWMonitor see if they are within specs for the card. Test the card in another system that can run it, see if it works there fine.

My graphics card seems to be a little above the stated speed but only by 0.4 ghz. I've checked for temperatures as well. Since I last posted things got better for a short while now they are far worse. The graphics card (I believe) is squealing and I'm hitting fps of 300 to something like 1000, I think I maybe saw 2000 once. The bands are still noticeable too.

Currently I can't test the graphics card elsewhere.

Is there something else I might be missing or am I better to just call it and try a new graphics card?
