Is It possible to replace hard drive without losing data ?

So I have ran into an error, sadly my solution to this error is to replace my hard drive, but My old HD have all of my work file, programs and Games. So is it possible to replace my hard drive without losing data ?
 
Solution
You can clone your old hard drive if it is still readable. Clonezilla works really well and is free. Just need a same or larger hard drive to clone to.
If cloning is not an option, then you can put your old hard drive in a USB enclosure and then just copy what you need back to your new drive once you get it up and running. One trick is I use a key recovery program to pull the keys for your software from the old hard drive while attached via USB so you at least have those when you reinstall some of your software onto the new drive.
