New member! Weird problem with GTX 960

Hi, new to the forum! I'm hoping someone here can help. I just built a new gaming PC, and I'm having a weird problem with my GTX 960 video card. When I first installed the driver, the screen went black and I thought the GPU was dead. So I started messing around with things a little, and after disconnecting my monitor from the card and plugging it back in, it started displaying video perfectly and working great! However, I have to reconnect the monitor every time I turn the computer on or wake it from sleep, as if it's defaulting to another video port that's not in use.

Does anyone have any ideas on what might be causing the problem, and how to fix it? My monitor connects to the DVI output. If anyone can help, I will be very grateful. :)

Intel Core i5-6400
Gigabyte GA-Z170-HD3P (F5g BIOS)
G.Skill Ripjaws DDR4 2400 8GB (2x4GB)
Samsung Evo 850 250GB SSD
Asus Xonar DS audio card
EVGA 500W power supply
Windows 10 Pro 64 bit

It's pretty moderate for a gaming PC. I've never been one to spend thousands on the latest and greatest. Whatever will get me by with what I want to use it for is good enough. I've been using a gaming PC with a Q6600 and 8800GT for the past 7 years or so. :)

Thanks for getting back to me! The Intel graphics don't show up in the Device Manager, so it seems it's been overridden by the Nvidia graphics.

Sorry, forgot to mention the monitor. It's a Samsung S24A300B. I've been using it with my 8800GT and Windows 7 for a few years now with no problems.

Yup, DVI cable. I don't have a VGA cable readily available, so I haven't been able to try that (PNY did provide an adapter).

I just tried uninstalling and then using older drivers, but didn't get any change. Updated back to current drivers.

Set the monitor to DVI only, still no change. The video card is in PCI-E slot 1 and is defaulted in the BIOS to that setting. I've also tried disabling Intel video within the BIOS.

When I have the Nvidia driver uninstalled and the video card running off Microsoft's standard VGA adapter driver, it immediately sends video to the monitor. I've searched for other settings within the motherboard's BIOS and within Nvidia's control panel, and haven't found anything yet.

After doing a Google search, I've found this problem has come up before with Nvidia cards, but I didn't see any actual solutions. One other thing I've noticed is that when I unplug the monitor, it gives the little ding that happens when a USB device is disconnected, and then dings again when I plug it back in. I'm wondering, is it possible this is actually a motherboard chipset driver issue?

I was afraid that might be the problem, but I thought I'd check into other solutions first. The card was an open-box item, so that may be why. Oh well, at least the seller offers free return shipping!
